Foods for Special Medical Purposes (FSMP) Notification Form

Overview

This form must be submitted when a notifier wants to place an FSMP on the local market. The notifier must submit contact details as well as information on the FSMP. The product label together with proof of payment must also be submitted.

What you'll get

Once the form together with the required documents are submitted, all documents are reviewed. If all documents submitted are correct a notification receipt is issued. The process takes a minimum of 2 weeks.
