VEH 39 - Declaration Form

Overview

The VEH 39 - Declaration Form provides the general public with a service to declare lost items related to their vehicle, driving documents, and tags. Following vetting of the application and client’s payment, Transport Malta can re-issue the lost item/s.


This e-form excludes vehicles with a 3rd plate. 

What you'll get

Reprint of either:
1.    Driving licence, 
2.    Re issue of vehicle number plate/s, 
3.    Reprint of registration certificate / logbook of vehicle, 
4.    Reprint of vehicle road licence,
5.    Reprint of Driver Tags.

 

Eligibility

All drivers licensed with Transport Malta and any person who has a vehicle registered on his/her name.

How to apply

Fill inthe online form and submit. 

 

Documents Required: 

 

Logbook and ID Card are to be presented upon collection of requested items. If logbook has been declared lost, ID Card is to be presented to collect the requested items.
