Application form through which authorisation holders/registration holders of any veterinary medicinal products authorised through any authorisation/registered route can notify the Animal Health and Welfare Department with the intention of withdrawing the products from the Maltese market.

How to apply
Fill in the online form and submit.
Required documents:
i) The latest authorisation for the product granted by the Veterinary Medicines section. This may be returned via post.
