Please attach the following documents:
- Copy of Passport or Residence Permit Certificate (if applicant is not of Maltese nationality)
- Copy of Work Permit issued by JobsPlus
- Passport-size photo of the Applicant
- No Objection declaration by the Environmental Health Directorate in the case of sale of food and beverages
