Declaration for a Loss of a Registration Certificate for Small Ships

Overview

An applicant is to fill in this form when applying for a re-issue of a lost registration certificate for a small ship registered under his/her name.

The re-issued document is to be returned if the lost document is found.

Persons wanting to obtain a reprint of a lost certificate.

How to apply

Print and fill in the form and send to:

Small Ships Register
Ports & Yachting Directorate
Transport Malta
Triq Pantar
Lija LJA 2021

Together with:
i) A copy of the ID card (front and back).
ii) A cheque payable to Transport Malta for the amount of €10.
