Overview
This form must be submitted for every natural person that is a director, shareholder, CEO, Ultimate Beneficial Owner, and key persons of the Operator/Licensee as per applicable laws and regulations and/or for any other person that the Authority may request.
What you'll get
The application is processed to confirm that the proposed applicant is fit and proper to be involved in the applicant’s operations. If all the requirements are satisfied, the Malta Gaming Authority (MGA) issues the relevant approval. An updated Personal Declaration form needs to be re-submitted if the form in respect of the respective Operator/Licensee held by the MGA is older than 5 years.
Eligibility
The information together with the supporting documentation submitted with respect to the applicants are used to conduct probity checks on whether the applicant is fit and proper as required by Law.
