Required documents:

i) A power of Attorney document (if representative is appointed).

ii) In case a representative has not been appointed and owner of the Patent is an individual, attach a copy of the identity card or passport.

iii) In case a representative has not been appointed and owner of the Patent is an company, attach a decleration issued by the company certifying the request. 

iv) An agreement between the licensor and licensee to authorise granting or cancellation of the licence according to the purpose for which this application is being filed.
