Fill in the online form and submit.
Required documents:
i) A site plan (1:250)
ii) Curriculum Vitae of the managing pharmacist
iii) Standard Operating Procedures (SOP)
iv) Premises plan (1:100)
v) Proposed layout of storage areas and dispensing area
vi) Police conduct of applicant
vii) Planning authority Permit for a shop
viii) Malta Financial Services Authority (MFSA) License (if the applicant is a limited company)
ix) Memorandum and articles of company (If the applicant is representing a company).
x) Proof of payment of the applicable fee.
