This application form serves as a declaration to the MCCAA when a new responsbile person(s) has been appointed for the lift, instead of the previous responsible person(s).

When two or more previously independent undertakings merge or one or more previously independent undertakings merge or one or more undertakings acquire control of the whole or parts of one or more other undertakings, the undertaking acquiring control in the case of an acquisition or the parties to the merger or to the joint venture is/are to file the Concentration Notification Form prior to the implementation of the concentration. The form has to be submitted within 15 days from the conclusion of the agreement, the announcement of the public bid, or the acquisition of a controlling interest.

This form must be submitted when a notifier wants to place a food supplement on the local market. The notifier must submit his/her contact details as well as the information related to the food supplement. The product label together with proof of payment must also be submitted.

The Market Surveillance Directorate is concerned with the safety of products used by people during the course of their life. With respect to this, the Market Surveillance Complaints Form allows one to file a complaint on the safety of products. These are divided into the following areas:

- children (eg safety of toys, playground and other items used by children);
- domestic and occupational (eg fire safety of electrical appliances, safety of DIY tools and communication equipment and safe use of domestic chemicals, safety of lifts);
- recreation (eg safety of engines used on recreational craft, media equipment and equipment used in amusement parks);
- personal care (eg s
afety of cosmetics and tattoo dyes)
- industry (eg safety of machinery, pesticides and fireworks)

This form serves as a notification of biocidal products containing active substances which are still under review in accordance with the transitional measures stipulated in Article 89 of the Biocidal Products Regulation (BPR, Regulation (EU) 528/2012) and LN 348 of 2013.

This is application form is intended for the certification as a professional user and/or distributor of plant protection products after having successfully followed the appropriate initial/additional training by bodies designated by the MCCAA. For your reference, training bodies designated by the MCCAA are made public through the MCCAA website.

In this form it is required to provide applicant details, such as contact details, details of the course followed by the applicant, as well as the certification type required by the applicant depending on whether the application is intended for certification as a professional user or as a distributor, and whether certification requires the granting or renewal of a certificate. The applicant is required to attach a copy of his/her passport photo in the application form.

This eForm targets Private Ltd. Companies registering for the first time for:

1) VAT number.
2) A PE number, in case where the applicant intends to engage employees during initial stages.

This form is to be used for the registration of lifts installed before the 1st of July 2002 with the Authority, as per the Inspection of Lifts Regulations, LN 231 of 2007.
