Annual return to Radiation Protection Commission
Radiation employers who issue dosimeters to their workers need to supply the annual doses received by their workers to the Commission by the 30th of April each year
Employer’s Notification to appoint a Radiation Protection Expert
An employer who has a legal obligation to consult a Radiation Protection Expert , has to notify the Radiation Protection Commission of the person that the employer will be engaging as Radiation Protection Expert (in compliance with SL 585.01).
Importing Agent Notification Form
Any Agent who intends to import sealed or unsealed radioactive sources into Malta on behalf of an authorised user, needs to seek approval from the Commission prior to shipment.
Medical Physics Expert Approval Application
In accordance to SL585.01, a person who wishes to act as Medical Physics Expert has to be approved by the Radiation Protection Commission. This form acts as the 'application' form for such approval.
Notification on the use of Radiation Sources
This notification is required by all users of ionising radiation in accordance with SL 585.01.
Notification to carry out off-site Work
This notification is required by authorised users of ionising radiation if work is to be carried out in other sites other than the authorised workplace.
